The NWD Integrative Screener includes the following heath domains:
- Physical Health/Medical Concerns: general health, pain, disability, pregnancy/prenatal care
- Mental Health: depression, anxiety, trauma, suicide, resiliency factors (e.g., self-esteem)
- Substance Abuse: alcohol use, tobacco use, illicit substance use, non-prescription use of prescription medications, sex/drug-linked behavior
- Infectious Disease Risk: incarceration, injection drug use, needle sharing, risky sexual behavior (e.g., sex/drug-linked behavior, multiple sexual partners, inconsistent condom use)
- Infectious Disease Testing History: Gonorrhea, Chlamydia, Syphilis, HBV, HCV, TB
- HIV Testing History and Treatment: most recent HIV test date and result, treatment adherence
Adaptable
The NWD Integrative Screener is flexible:
- It can be completed as an interview between a provider and client or self-report measure
- It can be completed electronically through a Microsoft Excel® program or manually (paper-and-pencil)
- A completed screen can be done in only
- 15 – 20 minutes
- Can be fit to meet individual agency needs
